In order to enforce child support payments in Utah a parent not receiving child support funds owed to them may be able to contact the Office of Recovery Services Child Support Services (ORS/CSS) for assistance. ORS/CSS may be able to help locate the other parent, if needed and automatically have funds deducted from their paycheck on a weekly/biweekly/monthly basis so that the funds are received.

How to File a Complaint

In Utah, a parent is obligated to pay child support in accordance with any child support validly issued by a court either in Utah or in another state.  Child support orders are enforced across state lines by the Uniform Interstate Family Support Act.  Child Support payments must be remitted as specified in the court order.

In order to have a child support order enforced, a parent must first fill out an application with the ORS/CSS.  The application can be filled in online or on paper.  All documents should be sent to the following address:

How to Fill-in

Step 2: Read the first page of instructions and on the next page enter the following information about the applicant:

  • Name
  • Date of Birth
  • Social Security Number
  • Relationship to the Children
  • Gender
  • Are YOU the mother or father of the children listed on this application?  If “Yes,” skip to “Section II Release of Information”

Step 3: Check whether there is a domestic violence concern

Step 4: Enter the following information about the mother first and then about the father on the next page:

  • Name
  • Current Marital Status
  • Driver’s License
  • Current Spouse/Significant Other’s Name
  • Social Security Number
  • Age
  • Other names, nicknames or aliases the parent has or may be using.
  • Date of Birth
  • Place of Birth
  • Is the parent deceased?
  • Date and Place of Death

Step 5: Enter the following information about each child:

Step 6: Enter the following information about the relationship of the parents:

What ORS/CSS Can Do

The Office of Recovery Services/Child Support Services (ORS/CSS) uses a variety of enforcement actions to collect past-due support, including, but not limited to: lien-levies, property executions, civil contempt, supplemental proceedings, tax refund intercepts, and criminal nonsupport.
