Enforcing child support payments in Arizona requires a parent to provide information about the non-paying parent, the children, and the child support order to the Arizona Division of Child Support Services (“DCSS”). The DCSS will then review the information and seek to have the order enforced.

How to File a Complaint

In order to file a complaint, a parent should provide as much information as possible about the non-paying parent, the child support order that is being violated and the children covered by the child support order.  The more information that a parent can provide, the better DCSS will be able to use their resources to enforce the order.

How to Fill-in

Step 2 – Fill out the following information on Section 1:

  • Your Name
  • Your Date of Birth
  • Your Social Security Number
  • Whether  you are the father, mother or caregiver
  • Whether you have an attorney

Step 3 – If you are the caretaker of the children, fill out the following information about you (Section 2) and the mother (Section 3) and the father (Section 4). If you are the mother or father, fill out the following information about the mother (Sections 3) and the father (Section 4) as follows:

  • Full name
  • Any previous names
  • Social Security Number (SSN)
  • Place of birth
  • Date of birth
  • Enter whether a member of a tribe and if so the tribe information
  • Residential address and mailing address
  • Phone numbers
  • Email address
  • Occupation
  • Employer name and address and phone number (Sections 3 and 4)
  • Enter whether the person has ever applied for child support, medical assistance or cash assistance.
  • Enter whether the person consults an attorney regarding the children
  • Enter whether there was domestic violence involved
  • Enter the physical description of the mother (Section 3), the father (Section 4)
  • Enter the marital status
  • List previous marriages
  • List names of all children and names of fathers (Section 3) or mothers (Section 4)
  • Enter whether the person has ever lived in Arizona
  • Enter information about the vehicle
  • Enter whether the person has been incarcerated
  • Enter the Education level achieved.

Step 4 – Fill in Section 5 entering the following information for each child

  • Child’s full name
  • Child’s social security number
  • Whether the child lives with the applicant
  • Relationship of applicant to child
  • Date and place of birth
  • Place of conception
  • Gender
  • Race
  • Ethnicity
  • Whether the child is a member of a tribe
  • Relationship of child’s parents as birth
  • Whether a father is listed on the child’s birth certificate
  • Information about any court order that is in existence for this child

Step 5 – The form then must be filed at any of the DCSS offices.

What the DCSS Can Do

In the event that the non-custodial parent fails to pay child support as set forth in either a judicial or administrative child support order, the DCSS has options of enforcing child support orders, including administrative income withholding orders, state tax refund offset, asset seizure, credit bureau reporting and other remedies.
