To obtain an application online, go to the Colorado Child Support Services Application page and follow the instruction posted there:

All three methods require the applicant to provide a signed, hard copy of the application to the appropriate county, along with the requested supporting documentation, and the $20 application fee.  To find the nearest Colorado Child Support to your location, please visit the Colorado Office of Economic Security website and use the Office Locator feature which can be found on the lower, right hand side of the webpage.
