If a parent is seeking to have a child support order enforced against another parent for the payment of child support arrearages, they first need to open a case with the Colorado Division of Child Support Services.

How to File a Complaint

In order to file a complaint, a parent needs to open a case by either using the online application process, printing out an application or going to their local child support office to obtain an application.

How to Fill in the Application

Step 1 – On the first page, enter the county in which you will be applying for services

Step 2 – On the next two pages, read and initial each statement which outlines your responsibilities and Colorado Child Support Enforcement responsibilities.

Step 3 – On page 4, enter the following information as the custodial parent:

  • Name
  • Social Security Number
  • Date of Birth
  • Place of Birth
  • Address
  • Phone numbers for home, work business and cell
  • Email address
  • Employer name and address
  • Occupation
  • Whether it is ok to contact you at work
  • Your relationship to the children
  • How you obtained custody
  • Whether there is an attorney involved, and if so, name and address
  • Whether and where the children are on public assistance
  • If you are the mother, are you pregnant and who the father of the unborn child is
  • Emergency Contact information

Step 4 – On page 5 enter the following information (as much as you know) about the non-custodial parent:

  • Name
  • Relationship to children
  • Social Secuirty number
  • Date of Birth
  • Gender
  • Place of birth
  • Current or last known address
  • Phone numbers for work, home and cell
  • Email address
  • Employer and employer address
  • Occupation
  • Physical description
  • Race
  • Whether in prison and if so, where
  • Whether in the military
  • Whether disabled
  • Assets
  • Vehicle make and model
  • Drivers license number
  • Other biological children
  • Parents names and address of non-custodial parent
  • Any other information to help locate
  • Emergency

Step 5 – On page 6, enter the following information for each child:

  • Name
  • Gender
  • Date of Birth
  • Social Security Number
  • City and State of Birth
  • County of Conception
  • Parents listed on birth certificate
  • Child support order number, county and state

Step 6 –  On page 7, enter the following information about the parent relationship status

  • Whether the parents were ever married and if so, when
  • Date spearated and divorced and when
  • Date of last contact with other parent
  • Whether and how paternity was established
  • Whether the non-custodial parent ever lived with the children in Colorado, where and when
  • Whether domestic violence is an issue
  • Whether there is a restraining order

Step 7 – On page 7, enter the following information about medical insurance

  • Check whether the child is on Medicaid
  • Check whether there is medical insurance other than Medicaid
  • List the name and contact information of the insurance company, if any
  • Name the policy number and group number
  • Name the subscriber and the relationship to children (person who provides insurance coverage)

Step 8 – On page 8 – fill out the income and expense affidavit as follows:

  • Your name
  • Your address
  • Your social security numer
  • Your date of birth
  • Your phone number
  • Your employer information
  • Your monthly gross income
  • Your adjusted gross income
  • Number of children you are obligated to support
  • How much time the children spend with you
  • Enter any credits

Step 09 – Sign in front of notary public

Step 10  – Bring the application, a check for $20.00 and the following to your local office:

  • Copy of a state issued birth certificate and social security card for all children
  • Copy of personal identification (i.e. driver’s license)
  • A photo of the other parent, if available; it will be returned to you
  • Verification of your income (i.e., pay stubs, tax returns)
  • Copy of Marriage Certificate (if not available, supply date of marriage and/or other evidence of marital status)
  • Copy of Court Orders signed by a judge or magistrate (if not available supply date, county, state of filing and court case number):
  • Complete payment records of all support paid to the custodial party directly, through court, or a state disbursement unit
